Appointment Request

Making the appointment is easy.  All clients must fill out the request below regardless of preferred method of payment.  Then you will fill out the forms even if you select an office session. The therapist will receive the forms and contact you concerning your appointment. All sessions are fifty minutes. Email is limited to five send and receive iterations. 

Please go to "make payment" and purchase the selected therapy session ONLY IF YOU DESIRE TO PAY IN CASH OR BY CREDIT CARD.
